Who sings this song? Ready to Go

I found a church singing a song called "Ready to Go" on YouTube and would love to find a download of the song. Can anyone help me?

Verse 1:
I know a place of wondrous beauty, in a land not far away.
And although I've never been there, it won't be long and I'll go to stay.
The streets are gold, the walls of jasper, a mansion there awaits for me.

Chorus:
