Who sings this song? Ready to Go

I found a church singing a song called "Ready to Go" on YouTube and would love to find a download of the song. Can anyone help me?

Verse 1:
I know a place of wondrous beauty, in a land not far away.
And although I've never been there, it won't be long and I'll go to stay.
The streets are gold, the walls of jasper, a mansion there awaits for me.
But most of all I'll see Jesus, the one who died to set me free.

Chorus:
I'm ready to go, to that place called heaven.
I'm waiting the call for me to come on home.
And I'll be free from trouble and heartache,
I'll sail away, no more to roam.

Verse 2:
I'll be free from sin's temptation, I'll never have a pain or a care.
I'll rest forever by the river, oh don't that make you wanna go there.
I'll see my friends and all of our loved ones, who are waiting on that shore.
They are singing glad hosannas, tears are gone, we'll cry no more.
