Dutch Hymn: "o, Heer Jezus, Kom"

This hymn is on a CD called "Samen te Dienen", and was last released in 2002 by Het Urker Mannenkoor 'Hallelujah'. The song was either written or arranged by W.S. Weeden, A. Pronk, and W.H. Zwart. The CD slip cover does not have the lyrics written in it. A translation of the title would be "O, Lord Jesus, Come", and the song is sung to the tune of "I Surrender All". Any help with this song would be greatly appreciated
