I found this in two places but this was the only one I could cut/paste. Someone had posted this on a prayer forum.
The song can also be found in a book...
THE COMPLETE BOOK OF HYMNS written by Wm J Petersen and Ardythe E Petersen and the song found on page 466. In the book verses 2 and 3 were reversed of the way shown below.
