Looking for a hymn: "Bring to this table"

The hymn starts off as:
Bring to this table bread and wine
Signs of our..

Chorus:
Raise up your hearts to our Lord present here

These are the only pieces I remember of it. Does anybody know the title of the song, the author or its lyrics?

The title is Bring to This Table. I don't know the composer though. I'm also looking for this hymn.
