Looking for "Gather 'Round the Supper Table" - Communion Song

Does anyone know/have this song, both words and music?
These are the only words I can remember.

Gather 'round this (the?) supper table for the bread we need today,

Bright the cup, the plate, the candles, white the linen cloth that's spread,
For the Savior wants to feed us, 'fore we perish on the way.
(This last phrase might be the second phrase where the xxxx are.)

Three verses. I think the "tune" is also used in another song but I don't know what that one is.
