Ancient of Days

I have been trying to track down a song that I think is entitled "Ancient of Days." The lyrics I remember (to the best of my ability!) are: Ancient of Days, God of the Ages, Alpha Omega, Beginning and End. King of all Days (I Think?), Reigning Forever, God you are worthy of praise.

The Chorus is: God you are worthy, Lord you are worthy, God you are worthy of Praise!

I'd like to know all the lyrics as well as who wrote it so that hopefully I could find the audio somehow.
