Trying to find the title and who sings the song with the following lyrics:

Ever since Jesus came in, His blood has washed all away my sins
Sweeter and sweeter He grows each passing day.
With His strong hand He is leading...
I'll make it Home some day.

Jesus is holding my hand, leading me to that fair land....

He will be holding my hand when I cross that bright golden strand.
My Lord will lead me all the way home!
