Can someone tell me the name and author of this song?

My church used to sing it a long time ago when I was a boy, and I barely remember the last part of the lyrics and melody in my head. It's something like this:

"When you fall on your knees,
He will give your heart ease,
He's always there,
And He'll hear, when you pray."

It was a choral response song that the whole church or choir sung after the main service prayer. I loved that short little chorus as a boy, and I hope someone can direct me to finding the lyrics or the song if you know it?
